Results showed that compared with baseline, 4-AcO-DMT significantly increased ratings on scales measuring mystical experiences, altered states of consciousness, anxiety/fear, sedation/drowsiness, stimulation/energy and intensity of experience; however it did not cause significant increases in ratings for dysphoria/depression or mental confusion. Additionally there were no statistically significant differences between the mean maximum change from baseline for heart rate or blood pressure observed after administration of 4′-acetoxy DMT versus placebo suggesting no significant cardiovascular toxicity associated with use at this dose level. In conclusion this study found that while 4′-acetoxy DMT induced strong subjective effects it did not have any negative cardiovascular side effects at doses lower than those typically used recreationally or therapeutically.

II. CHEMISTRY
- CHEMICAL STRUCTURE
4-aco-DMT is a molecule made up of two rings, an indole and a piperidine ring. The indole ring consists of six carbon atoms, each connected to two hydrogens and one nitrogen atom in the center. The piperidine ring consists of five carbons with alternating single and double bonds between them, each connected to two hydrogens. On the third carbon in this ring there is an oxygen molecule attached. In total 4-aco-DMT has 13 hydrogen atoms, three nitrogen atoms and one oxygen atom making it a relatively small but complex molecule which can bind to serotonin receptors in the brain giving it hallucinogenic properties when consumed.
- SYNTHESIS
The synthesis of 4-aco-DMT, or O-Acetylpsilocin, is accomplished through a two step process. First the precursor compound psilocybin is converted to its freebase form. This can be done by adding a base such as sodium hydroxide or potassium carbonate and stirring the solution for several hours before filtering off any insoluble material. The resulting solution can then be acidified with hydrochloric acid to precipitate out the freebase product as crystals from which it can be isolated and dried. The second step involves acetylation of this freebase product with acetic anhydride in an inert solvent such as dichloromethane at room temperature for between 5 and 10 minutes until all reactants have been consumed. The reaction mixture should then be neutralized using sodium bicarbonate before being extracted into an organic solvent such as ethyl acetate or chloroform. Finally, the product should be dried over magnesium sulfate, filtered off and recrystallized from ethanol/ether (1:2) to isolate pure 4-aco-DMT crystals that are ready for use.
